ENROLLMENT REQUIREMENTS
Below are the items that are required for student enrollment:

Birth Certificate

Proof of Residency, such as
- Property tax statement
- Utility bill/agreement
- Real estate contract
- Legal property description
- Rental agreement/receipt
- Telephone bill

Student Health Inventory

Immunization Records

Custody papers, including a copy of the dissolution decree that stipulates custody in cases of parental divorce.

Records from Prior School (if transferring)
Parents or legal guardians will need to fill out a transfer request to receive records from the prior school. Parents or legal guardians can bring documentation in to expedite the process. If parents bring unofficial transcripts in, students can begin classes immediately, otherwise, students need to wait until the prior school sends records.
